For patients with dates of service March 1, 2021 and forward (excluding Dean Clinic locations):
All professional services are billed by Zotec Partners (Zotec). To speak with one of Zotec's customer service representatives, please call (866) 863‐6739 (7:00 AM to 3:30 PM CST). Please note: if you do not have your account number please enter 0 each time you are prompted for an account number. You will then be transferred to a representative.
To make a payment, check account balance, request a statement, update insurance information or to set up a payment plan, please go to Zotec's Patient Portal at http://www.mydocbill.com/rria. Patients may also make a payment over the phone at (866) 863‐6740. (Available 24 hours a day, 7 days a week).

For billing records requests, the most efficient way to obtain copies of billing records is to contact ChartSwap at 855-879-7927 or via their website at Chartswap.com.
Exception: Exams performed at Dean locations are billed as one invoice by Dean Health System (for both the radiologist and technical components of your bill). For questions regarding these bills, please contact Dean Health System at (608) 250‐1593.
For hospital or clinic charges, please contact the billing department at the location where your services were provided.
Medical records are maintained by the clinic or hospital where services were received. Please contact those facilities to obtain medical records.
