The problem of chronic or resilient hip pain in patients with normal x-rays is common. A variety of entities including: avascular necrosis, muscle tears, lateral / gluteal insertional tendonitis, ileopsoas tendonitis, and synovitis must be considered among others. Also commonly in this category, but more difficult to diagnose is tear of the acetabular labrum. In centers where hip arthroscopy is available, pre-operative evaluation with MR arthrography can be helpful in surgical planning.
